SEND support plan drafter

Draft a graduated-approach support plan — targets, strategies and review dates — from what you've noticed.

Describe what you have seen and what you've already tried, and the tool drafts a support plan in the assess–plan–do–review cycle with SMART-ish targets, adult strategies and a review date. It is a starting draft only: the plan belongs to your SENDCO, the family and the child, and nothing here is a diagnosis or a referral decision.

Use initials if you'd rather not type a name.

Facts and examples — when it happens, how often, what helps.

Their words carry weight in the plan.

No card needed — keep going as long as you like.

No child data is stored. Your notes are used to generate the draft and nothing else.

Getting the most out of this

  • The SEND Code of Practice expects a graduated approach: assess, plan, do, review — usually on a termly cycle, with parents involved at every round.
  • Two or three targets is plenty. A plan with ten targets is a plan nobody delivers.
  • Never record or imply a diagnosis. Describe need and provision, and route any referral through your SENDCO and local authority.
